Thank you for your interest in our service dog program. Applications are now open.

At this time, applications are being accepted for a Phase One service dog. Phase One is a 12–14 month program focused on building the foundation for the dog and the family to work together, including basic obedience, public manners, and the early stages of task development. During this time, families are also required to complete a handler training course while the dog is in training.

Our program works exclusively with Labrador Retrievers due to their temperament, trainability, and suitability for service work.

Following Phase One, the dog is placed with the family and training continues in the home as part of Phase Two, an “on-the-job training” phase. This includes continued daily work with the dog and monthly check-ins with the trainer to support progress and development.

Due to the high number of applications we receive and the limited number of placements available, not all applicants will be selected. Only those whose application is chosen to move forward will be contacted. We are not able to respond to every submission and appreciate your understanding.

If selected to move forward in the application process, additional steps will be required. These include a video of the inside and outside of your home, a phone interview with the foundation, a phone interview with the trainer, and a live “Getting to Know Your Child” interview. Each step helps ensure the program is the right fit for both the dog and your family. Completion of all steps does not guarantee approval; however, all steps must be completed to be considered for placement.

This program is supported through a financial grant and is based on demonstrated need, including proof of income.

A service dog can be an incredible addition to a diabetic care plan, but it is also a significant commitment. It is a long-term partnership that requires active participation from the handler and family. Ongoing training, consistency, and daily involvement are essential to maintain the dog’s skills and ensure long-term success.

Families should also be prepared for the ongoing financial responsibility of caring for a dog. This includes routine veterinary care, vaccines, grooming, food, toys, and we strongly recommend maintaining pet health insurance. Travel for service dog placement and in-person handler training in Buckeye, Arizona is also required and is the responsibility of the family.

At this time, applications are only being considered for diabetics between the ages of 7 and 12.
